Enrollment has been extended until 5:00 p.m. on Tuesday, November 2.
To enroll in Spirited Life:
Step 1: Indicate your interest by completing an enrollment form via the link below.
Upon receipt of this form, a Clergy Health Initiative staff member will confirm your eligibility and send you a unique username and password.
Eligible clergy include those serving under appointment to a local church or on conference staff in the North Carolina and Western North Carolina Conferences of the United Methodist Church. Individuals who participated in the Clergy Health Initiative pilot program have already received services, and are therefore not eligible for Spirited Life.
Step 2: Using your assigned username and password, log in to the secure portion of the Spirited Life website. The orange Log In button is at the top right of this website.
Once you are logged in:
- Review and complete the online consent form. A sample copy of the consent form is available for download.
- Sign up for and attend a baseline health screening in November 2010.
- Complete a baseline health survey during the first week of November. If you completed the Duke/Westat Clergy Health Initiative survey in 2010, a short supplemental survey will be all that is required to enroll in Spirited Life. Otherwise, you must complete the full survey.
Following completion of these enrollment steps, you will be randomly assigned to one of three Spirited Life groups to receive wellness services.
